If Your Vehicle Was Last Titled in New York
To replace a lost or damaged title in New York, you'll need to complete the Application for Duplicate Title (Form MV-902). You can download the form directly from the New York State DMV website.
Once you’ve filled it out, you have three options for submitting it:
1. Apply Online
You can submit your application online through the New York Department of Motor Vehicles.

2. Apply In Person
You can also bring your completed application to a local New York DMV office.

3. Apply by Mail

If Your Vehicle Was Titled in Another State
If your car was last titled outside of New York, you’ll need to contact the DMV in that state to request a duplicate title. New York can only issue replacements for vehicles originally titled in-state.
